Report of Outside Interests Related to Research (ROI) (revised 10/28/09)
This form is required if you are:
- submitting a research proposal or subcontract to Sponsored Projects Services (SPS) or to the Office of Research Contracts and Analysis (ORCA);
- beginning or requesting continuation of a study, either sponsored or investigator-initiated, that involves human subjects;
- reporting the addition of a new Investigator or other personnel to an existing project OR a new interest in the sponsor of an existing project OR a new subcontract/award related to an ongoing project OR a modification of an existing research project that significantly alters the scope of work.
Student Acknowledgment of Faculty's Outside Interests (revised 10/30/09)
This form must be submitted if a student has been requested, as part of his/her research and/or educational activities, to perform work on technologies owned or controlled by an outside entity.
Notice of Intent to Consult (revised 11/04/09)
If you will be entering into a consulting agreement with an outside entity, submit this form to your Department Head or Supervisor at least four (4) weeks prior to the start of your activities.
Annual Conflict of Interest and Conflict of Commitment Form
This form was created as a tool for internal departmental use; it is recommended that Department Heads or Supervisors request this information annually, between February 1 and March 1, to obtain updates regarding the status of employees outside activities for the year.
Enabling Disclosure Template (OTT form)
The Office of Technology Transfer (OTT) requires submission of this form if you:
- are creating a company or entity in which you will have an equity interest;
- will be involved in the corporate control of an entity that will or may enter into a contract with the University of Arizona;
- anticipate accepting a position with a company or entity as an officer, director, or other fiduciary position that has or will have a contract with the University of Arizona.
